What to Bring to your Appointment

New Patients:

Please fill out the new patient information packet for the physician you are scheduled to see (that you can download from this site) or we can mail you the set of documents in advance of your appointment. You can return the packet by mail or bring it with you to your appointment. If you have a more urgent appointment, please arrive 15 minutes early, in order to allow you time to fill out the various documents needed.


All Patients:

  • Kindly have your insurance cards and copayments (if applicable) ready at check-in. We accept cash, check and credit cards (Discover, American Express, Visa and MasterCard) for all payments due. Copayments are due on the day of your appointment. We do not bill for copayments after your date of service.
  • If an insurance referral is required from your insurance plan, please be sure it is received by our office prior to your appointment. We may not be able to see you if the proper referral is not in place for your date of service.
  • Please have a list of your current medications (including over-the-counter medications), including dosages and why they have been prescribed. Please also have a list of any allergies you have to medications.
  • Kindly bring your glasses. If you wear contact lenses, please bring a case and solution to store them for the examination.
  • Driver/sunglasses: New patients will nearly always have their eyes dilated. Most returning patients will have their eyes dilated, depending on the reason for their examination. Please bring a dark pair of sunglasses and – if you are very sensitive to light or are concerned about your vision after the exam – have a driver come with you.
